Course Provider Information

As you know, the FEF has partnered with the College Board to offer SAT preparation courses at our Centers of Excellence and at churches, community centers and other community-based venues around the State. Below you will be able to download the forms that will enable you to conduct your SAT course. If you require further information, please e-mail Lyra Logan or call her @ (813) 272-2772:
 

COURSE PROVIDER FORM DOWNLOADS:

As you know, the SAT Preparation Project Guidelines (see the Guidelines below) call for completion of the following forms in order to offer the SAT course. You need Adobe Acrobat Reader to download some of the forms. If you do not have Adobe Acrobat Reader, you may download it here.
SAT Course Application
Use this form to apply to offer your SAT Prep course. Complete a separate form for each course that you will offer. Fax completed forms to the FEF at (813) 272-2784. We will contact you after we receive your application.
Pre-test/Post-test Score Report
Use this form to report student scores on a pre-test given near the start of each course and on a post-test given on the last class session of each course. Email this form to Monica the business day after the class session during which a test was administered.
 
Daily Student Roster (Microsoft Excel file)
Use this form to report attendance at each SAT Prep class session. Have the course instructor complete the top portion and each student sign in and fill in his/her personal information each day. Then fax the completed form to the FEF at (813) 272-2784 on the next business day after each class session.
Stipend Request Form
Use this form to request stipend payment after your course is completed. Fax this form to the FEF at (813) 272-2784 after all course requirements have been met. If you are requesting payment for an individual, please have that person complete an IRS W-9 Form, and fax it along with your Stipend Request.
 
COURSE GUIDELINES

1. Course Curriculum Outline. For each course, be sure to have teachers follow the 25-hour Course Curriculum Outline which is provided with the Instructor Orientation materials. If you do not have a copy of the Curriculum Outline, please call (813) 272-2772 to receive one via fax.

2. Supplies. The course teacher’s manual is The Official SAT Teacher’s Guide, which contains all course lessons and exercises/handouts that need to be copied for use. The FEF will provide one Official SAT Teacher’s Guide for each course provider that has not offered an FEF course in prior years.

Instructors will find supplemental review materials and the required pre-and post-tests in the SAT Preparation Booklet and The Official SAT Study Guide. The FEF will provide one copy of both publications for each course provider that has not offered an FEF course in prior years.

3. Daily Student Rosters. Course instructors must have all attending students complete the College Board’s Student Roster at each class session, have each roster faxed to the FEF at (813) 272-2784 the business day after each session, and, at the conclusion of the course, send the FEF the originals of all Students Rosters for the course. 4. Pre- and Post-Tests. The Course Curriculum Outline requires teachers to administer and score the pre- and post-tests so we can gauge student progress. For the pre-test, use one of the tests in The Official SAT



Teacher’s Guide, and administer the same pre-test to all students. For the post-test, use a different test in The Official SAT Teacher’s Guide, and administer the same post-test to all students.


5. Pre- and Post-Test Score Reports. Course instructors must keep record of all student pre- and post-test scores and have reports of those scores emailed to Monica at the FEF the business day after the class session during which a test was administered. 6. Instructor Time & Effort Logs. Each course instructor must keep record on a time and effort log of the number of hours s/he works related to the course. At the conclusion of the course, the instructor must send the FEF his/her completed Time & Effort log along with all other required documentation for the course. 7. Course Stipends. The FEF will pay a standard stipend for each approved course offered that enrolls at least 10 students at each class meeting session. The stipend may be used to defray costs of paying instructors, making copies of course materials, purchasing additional course supplies, publicizing the course, and providing student refreshments and incentives. The stipend is paid in one installment at the conclusion of the course, after the FEF verifies that all course requirements have been met and receives a completed stipend request form.
